Student Forms

Students must submit both the Upper Division Honors Application to science@lsu.edu prior to the start of their Junior year. Students must receive Department approval prior to submitting the Upper Division Honors Application to the College of Science.

Second Discipline Elective Forms

• Chemistry Second Discipline Electives (Catalog years 2022 or earlier)
• Mathematics Second Discipline Electives (Effective: 2016-17)
• Physics Second Discipline Electives

Students must receive Department approval prior to submitting the Secondary Discipline forms to the College of Science.
